What is the the vote of no confidence and how do you get it?

If you're a *really* terrible queen, at the ball during the toasts your abilities may be called into question and there'll be a vote about whether you're good enough to become queen.

My advice: study things that aren't very useful during the early game (I did mostly dancing), and try to do as bad as possible (i.e. upset the most people) with the ixion negotiations.

No matter how hard / try I can't get this event.

Make Banion mad (like by giving away territory to Ixion) then when he calls you on it at the gala, try to have him executed.
